Carol D. Cook's Obituary
Carol D. (Kurtz) Cook, age 60, of Collinsville, IL, died June 10, 2006, at Anderson Hospital in Maryville, IL. Visitation will be held from 4 PM to 8 PM on Tuesday, June 13th, at the funeral home in Collinsville. Funeral services will be at 10:30 AM on Wednesday, June 14th, at Good Shepherd Lutheran Church in Collinsville, IL. Burial will be in Holy Cross Lutheran Cemetery in Collinsville, IL.
