Douglas L. Sullivan, 40, of Collinsville, Ill., born July 21, 1971, in Belleville, Ill., died Friday, August 19, 2011, at his residence.



Mr. Sullivan was preceded in death by his mother, Nina, nee Starnes, Sullivan.



Surviving are his father, Malcolm "Presley" Sullivan of



Collinsville, Ill.; two brothers, Donald Sullivan of Colorado Springs, Colo., and David Sullivan of Denver, Colo.; a sister, Diana Burk of Puebelo West, Colo.; and aunts, uncles, cousins, nieces and nephews.
